Job Description

Essential Functions:


Relationship & Stakeholder Management:

  • Serve as the primary liaison for assigned practices, coordinating communication across internal and external partners.
  • Engage cross-functionally with Clinical Operations, Implementation/Training, Data, Billing, QA/Compliance, IT/Integration, and Sales/BD.
  • Maintain a structured communication cadence (weekly, monthly, or quarterly as appropriate).
  • Manage escalations and coordinate timely resolution.
  • Ensure partner communications receive responses within 24 hours.
  • Translate partner feedback into actionable, prioritized tasks.


Contract, Scope & Customization Oversight:

  • Ensure internal teams deliver services according to contracted scope (CCM, CoCM, RTM, and adherence programs).
  • Identify and escalate potential scope changes for internal review and pricing.


Program Implementation & Optimization:

  • Support onboarding and expansion of new practices and service lines.
  • Participate in workflow mapping exercises to align current-state and target-state processes.
  • Monitor referral volumes and identify potential barriers to enrollment or throughput.


Performance Measurement & Insights:

  • Convert Excel-based operational and clinical data into meaningful insights for partners and internal teams.
  • Communicate trends, risks, and opportunities to support decision-making and performance improvement.


Project Management:

  • Maintain a partner project tracker with open items, owners, due dates, and status updates.
  • Support cross-functional follow-up to ensure progress against project timelines.


Quality Assurance & Risk Management:

  • Collaborate with Clinical Operations on audits, root-cause analysis, and corrective action plans.
  • Track patient-experience issues reported by practices and coordinate timely remediation.


Retention & Growth Support:

  • Maintain accurate, up-to-date partner profiles.
  • Identify under-utilized services and recommend opportunities to strengthen engagement.


Communication & Internal Alignment:

Conduct regular alignment huddles with Clinical Operations and Billing to review:

  • Escalations.
  • KPI performance.
  • Implementation tasks.
  • Operational risks.


Required Education and Experience:

  • Bachelor’s degree in business administration or a relevant field.
  • Proven experience developing strategies for value-based care in a changing healthcare market.
  • Knowledge of risk contracting and population health operations.
  • Experience in healthcare or Population Health fields.
  • Experience working with Accountable Care Organizations and risk-sharing models.
  • Proficiency in Microsoft Office 365 and related suites.
  • Well-organized and responsible with strong problem-solving abilities.
  • Excellent verbal and written communication skills.
  • Team-oriented with a high degree of dedication.

Position Type and Expected Hours of Work:

This is a regular, full-time position that operates in an office or remote work environment. Some travel may be required.

Physical Requirements:

This is a remote position that requires the ability to sit or stand for extended periods and work at a computer.
